Understanding the Difference Between Two Medical Emergencies
Stroke is a disease that affects the arteries leading to and within the brain. It is the fifth-leading cause of death and a leading cause of disability in the United States*. "Stroke" and "heat stroke" sound alike, but they are two very different medical emergencies – and confusing them can waste critical minutes when every second matters.
What is the difference between stroke and heat stroke?
Think of your body as a machine:
- A stroke is like a sudden “plumbing” problem. A clot blocks blood flow inside the brain (this is considered an ischemic event).
- A heat stroke is like an “overheating engine”. Extreme outside temperatures cause your body's internal systems to fail (this is considered a hyperthermic event).
One crucial question separates the two: "Was this person just exposed to intense heat? The answer helps you – and emergency dispatchers – quickly identify whether you're dealing with a neurological emergency or a heat-related one, so the right kind of help arrives fast.
Traditional stroke: Use B.E. F.A.S.T.
A stroke cuts off blood supply to the brain, causing the body's electrical signals to misfire. These misfires typically create sudden, observable physical symptoms called neurological deficits – and they can happen to anyone, regardless of the weather. Use the B.E. F.A.S.T. acronym to recognize the signs: Balance – Sudden loss of balance or coordination? Eyes – Sudden vision loss or double vision? Face – Ask them to smile. Does one side droop? Arms – Ask them to raise both arms. Does one drift down? Speech – Ask them to repeat a phrase. Is it slurred or odd? Time – Call 911 immediately if you see any of these signs. Every minute matters: millions of brain cells can die while blood flow is blocked. Fast medical care is the single biggest factor in helping to prevent permanent damage or disability.
What are the warning signs of heat stroke?
Heat stroke is a different crisis entirely. Instead of an internal blockage, it happens when your body's cooling system breaks down under extreme outside heat. High temperatures and humidity stop sweat from evaporating, trapping heat inside the body until it becomes dangerous. Watch for the shift from ordinary heat exhaustion to a life-threatening emergency:
- Dangerously high core temperature – body temp rapidly climbs above 104°F
- Hot, dry skin – sweating stops as the cooling system fails
- Rapid pulse – the heart races to push heat to the skin
- Altered mental state – confusion, agitation, or fainting, signaling brain swelling
Call 911 immediately. While waiting for help, work to lower body temperature using cool, wet cloths and shade.
Who's at higher risk for stroke and heat stroke?
Traditional stroke risk factors:
- Previous stroke or transient ischemic attack (TIA)
- High blood pressure
- High cholesterol
- Heart disease
- Diabetes
- Sickle cell disease
Heat-related illness risk factors:
- Older adults (65+)
- Infants and children
- People with chronic conditions
- People without access to air conditioning
- Outdoor workers
- Athletes
- Pregnant women
